CRIMINAL RECORD CHECK
Under the New South Wales Government Legislation, those who will be undertaking one or more of the following postgraduate courses should be informed to complete forms 1 and 2.
Master of Chiropractic
Master of Clinical Audiology
Master of Clinical Neuropsychology
Master of Clinical Psychology
Master of Organisational Psychology
Master of Social Health
Master of Speech and Language Pathology
Postgraduate Diploma in Applied Psychology
Postgraduate Certificate in Applied Psychology
Postgraduate Diploma in Social Health
Postgraduate Certificate in Social Health
If you have already obtained a Criminal Record Check clearance for your current course, you do not need to fill out the Criminal Record Check Consent form.
If you have already completed a Prohibited Employment Declaration Form you do not need to complete this form.
Complete and submit the form(s) required in person to the Student Enquiry Service, Level 1, Lincoln Building, Macquarie University as soon as possible and no later than the final date for your enrolment.
